Sounds like your axle was swapped in at some point? An IH axle only uses three ubolts on the front axle. Either you have a 74/75 axle if it has external locking hubs or a Chevy axle. The suspension kit doesn't care what axle you have, as long as you get the correct ubolts and plates to make it work.
 
Apologies, lack of sleep. Yep, the axle (73.5-75) was swapped in to my truck at some point in its life. Just wanted to make sure there wasn’t anything besides the missing extra u-bolt to worry about.

Will the plates that come with the IHPA kit work on the 74-75 disk axle?
 
We build the kits per orders placed. I don't believe we have any shortages at this time. We do however sell a million things a day, stock changes rapidly. We have plenty of the ubolt plates.

You also need another ubolt: https://www.ihpartsamerica.com/store/cpt-ubolt-5-8.html - the 3.75" one to go around the housing. The ones we have now are 3 5/8", so it is going to be a tight squeeze. You may also need to die grind the slots in the housing to accommodate the larger ubolt.
